위암유전, 위암의 표적치료 타겟 발굴 및 동반진단법 개발
1. 위암은 전세계적으로 암 사망의 두 번째 원인으로, 연간 989,000여 명이 새로 이환되고 738,000여 명이 사망하는 소화기암 분야에서 가장 중요한 관심의 대상임. 위암은 진행된 후 발견되는 경우 고식적 항암치료 시 평균 생존기간은 11-12개월 정도로 예후가 매우 불량함. 2. 위암에서 발견되는 유전자로는 TP53, CDH1, ARID1A, APC, PIK3CA, CTNNB1, KRAS, MSH6 등이 있음. 3. 본 연구자는 PIK3CA가 전이성 진행위암에 흔하고, CTNNB1이 EBV 연관 위암에 흔하며, ROS1-SLC34A2 fusion이 위암에도 존재함을 처음으로 밝힘. 4. 표적 치료제는 기존의 고식적 항암제보다 비교적 부작용이 적은데다, 일부 표적 치료제들은 기존 항암화학요법과 병용 시 생존율을 증가시킨다는 고무적인 연구 결과들이 많이 발표되고 있음. 5. 표적 치료제는 암이 형성되는 과정에 관여하는 특정 표적인자만을 공격하므로 같은 종류의 암이라도 특정 표적인자가 나타나는 환자에게만 효과를 나타냄. 따라서 표적 치료제를 보다 효과적으로 사용하기 위해서는 표적 치료제에 대한 효과를 미리 예측할 수 있는 예측 지표를 정립하는 것이 필요함. 6. 표적 치료제를 사용하였을 때 효과가 높을 가능성이 있는 환자들을 똑바로 선택하는 것은 불필요한 의료비의 지출을 줄이고, 환자에게 나타날 수도 있는 불필요한 부작용을 미리 예방한다는 차원에서도 매우 중요함. 7. 또한 지속적 표적 치료제 투여 시 내성이 생기는 데, 이러한 내성 기전을 규명하고 예방하기 위한 연구가 필요 함. 8. 따라서 본 연구자는 위암 타겟치료의 적응증이 되는 환자를 정확히 찾아내고, 새로운 표적치료 타겟 및 예후인자 발굴, 그리고 동반진단법 개발 등의 연구를 활발히 하고 있음.
