만성 심방 세동 환자들을 의한 하이브리드 술식
만성 심방 세동은 뇌졸중, 심부전, 혈전증 등 심각한 합병증의 원인이 되지만 매우 흔한 질병 (65세이상 2%가 경험) 중의 하나입니다. 하지만, 약물 치료는 한계가 있고 가장 효과가 있다는 고주파 절제술도 재발이 흔해 현재 난치병 중의 대표적인 질환입니다. 가장 효과가 있는 수술적 치료는 절개가 클 뿐만 아니라 수술 자체가 가지는 위험성으로 다른 심장 수술과 동반되는 경우에 시행될 뿐 단독으로는 시행되고 있지 않습니다. 최근 흉강경을 이용하여 부정맥 수술 (양극성 고주파 절제) 을 시행하기 시작하였고 보다 완전한 성공율을 위해 경피적 고주파 절제술 (내과) 도 같이 시행하는 하이브리드 술식이 개발되었습니다. 다시 말하면, 하이브리드 술식이란 흉강경 수술을 통해 심장 외부에서 심방 세동의 원인이 되는 부분을 차단하고 동시에 심장 안쪽에서 고주파를 이용하여 비정상적 신호를 유발하는 부분을 또다시 차단하는 이중 차단의 효과를 도모하는 것입니다. 현재까지 60여명에서 시행되었고 (국내 유일) 기존 술식에 비해 1년 성공율 (93%) 은 기존 술식들 (50~70%)에 비해 월등합니다. 기존 술식이 실패한 환자, 항응고 요법이 위험한 환자 등 적응증 자체는 넓지만 아직 보험 적용이 되지 않아 많은 분들에게 시행할 수 없는 실정입니다. 하지만. 곧 부정맥 수술의 표준으로 자리 잡을 수 있을 것으로 기대합니다. 하이브리드 술식도 아직 연구의 여지가 많습니다. 특히 정상 박동 전환 후 좌심방 기능 회복, 좌심방이 절제 범위 및 적응증, 우심방 차단 술식 등 100% 성공율을 위해서는 여전히 많은 발전이 필요합니다. 여러분의 소중한 관심 부탁드립니다. 연구 현황 및 계획) 동물을 이용한 냉동 소작기의 심장 외부 접근의 안전성 실험. 동물을 이용한 우심방 차단의 새로운 접근 개발 방식 고안.
