Medical Record
Please fill out the REQUEST FOR MEDICAL RECORD COPY and send it to us
- Patients are required to fill in a REQUEST FOR MEDICAL RECORD COPY at the IHS Center.
- The cost of issuance of the record will depend upon the number of pages. After payment, you will receive a copy of your Medical Record.
- The time taken to issue the Medical Record may vary depending on its size, but it takes usually 10 to 30 minutes.
- The patient’s photo ID is required to issue the Medical Record.
- A proxy of the patient may request the Medical Record by presenting the patient’s and the proxy’s IDs, the CONCENT on the MEDICAL RECORD REDING AND COPY and the POWER OF ATTORNEY on the MEDICAL RECORD REDING AND COPY, of which must be signed by the patient.

- Medical Records are issued from 8am to 6pm during weekdays, and from 8am to 1pm on Saturdays

Film Duplications
You may request copies of film or images (X-rays, CT, MRI, etc.)
- Patients can request a copy of the medical image data at the IHS center.
- The medical image data will be copied in the form of a CD. It usually takes 30 minutes on average, although the time taken may vary depending on the quantity of data.
- The cost of the image data will depend on the quantity. After payment, you will receive a CD of the data.
- Medical image data are issued from 8am to 8pm during weekdays, and from 8am to 5:30pm on Saturdays.

Medical Certificates
You can ask your doctor directly during the consultation, as the doctor will write it.
- Patients can ask for a referral and medical report in English, confirmation document of hospitalization and discharge, and confirmation document of medical treatment as an outpatient.
