Calcifications are identified on mammography, and additional magnified views are often taken so that the radiologist can study their exact shape and arrangement. Findings are categorized using a standardized reporting system known as BI-RADS, which estimates the likelihood that a finding is benign or potentially cancerous and recommends a corresponding course of action. Clearly benign calcifications need no further testing, while findings that appear probably benign are usually rechecked with a follow-up mammogram after a short interval to confirm they are stable. When a cluster looks suspicious, a tissue sample is obtained, most often through a stereotactic biopsy or vacuum-assisted biopsy, in which the calcifications are precisely targeted using mammographic guidance.

Most breast calcifications require no treatment at all and are simply noted and monitored over time. When follow-up imaging confirms that calcifications are stable, a patient continues with routine screening. If a biopsy shows the calcifications to be benign, no further action beyond regular screening is generally needed.

If a biopsy reveals that suspicious calcifications are associated with ductal carcinoma in situ or another form of breast cancer, treatment is directed at that underlying diagnosis and may involve surgery, radiation therapy, and other therapies determined by the specific findings. In these situations, the calcifications served their most useful purpose by allowing a cancer to be detected at a very early and highly treatable stage.
